Occupational Tax Certificate

To request an occupational tax certificate (business license) you will need to complete and return all three forms (links below) to City Hall.

Please keep in mind that two of the forms are affidavits and must be notarized.

• Occupational Tax/Business License Application
• Private Employer Exemption Form
• State of Georgia Affidavit
• E-Verify Affidavit
